Important Information for Your First Counseling Appointment

Your first appointment at Catholic Charities will be 50-60 minutes with a counselor, unless Client Care specifies a longer counseling session. Please bring all required documents to your first appointment to avoid the need to reschedule.

We have many services at Catholic Charities, some of those services require a fee. You will be notified at the time of scheduling if there is a fee associated with your service.

Catholic Charities does participate with many, but not all insurance companies (not Medicare or Medicaid).  Please have your insurance information available when you call to schedule services and to verify our participation with your plan. Please bring your insurance card as proof of insurance to your first appointment.

If you do not have an insurance plan that we participate with, your fee will be determined by a sliding scale.  Income verification will need to be provided at your first appointment to determine the fee for services.

Fees including insurance co-pays are due at the time of service and are payable in cash, check or credit card. Catholic Charities does not accept medical cards.

Income Verification

Fees are based upon an income-based sliding scale ranging from $10 to $100.

Please note that total combined household income and number of household members are both used to determine client fees.

Catholic Charities accepts the following forms of income verification:

Income Tax form with adjusted gross income
Two most recent paystubs
Social Security Award Letter/benefits
Disability/Unemployment Award Letter/benefits
Child Support

If income verification is not provided at your first appointment, you will be charged a $45 fee prior to meeting with a counselor or you may reschedule the appointment until income verification is provided.

No Income

If you receive no income, you will need to provide documentation such as food stamp voucher, a letter from a social worker stating “not employed” or “not receiving any income.” If you are living in a shelter, a letter from the shelter is required.

Client Care 859-581-8974 is available Monday through Friday, 9am to 5pm for assistance in answering questions related to insurance, income, and service fees.
