Client Care and Case Management

Catholic Charities, Diocese of Covington is accepting qualified candidates for a client care and case management position within the agency. The position is 60% (24 hours a week).

Job Requirements:

  • Bachelor’s Degree in a human service field
  •  Knowledge and understanding of community resources in Northern Kentucky
  • Ability to establish effective working relationships with internal agency staff as well as with partnering community organizations
  • Ability to complete comprehensive human service assessments based on face-to-face interviews
  • Strong work ethic, excellent with details, and a self-starter
  • Good problem solver and listener
  • Ability to communicate effectively, verbally and in writing, to maintain confidentiality
  • Desire to work as a team member and ability to work independently under general supervision.

Responsibilities of Client Care Position- 20%

  • Respond to live phone calls, walk-in visits and messages from persons seeking social services, assist applicants in clarifying their needs and determining if CC services are appropriate. Provide a positive, appealing, and uplifting first impression of CC.
  • Register applicants for CC services or offer appropriate referrals as needed.
  • Respond to requests for information and referral from other agencies or organizations
  • Manage the waiting list.
  • Record in the case management database all information regarding requests for service and the outcomes of those interactions.
  • Assign clients to service providers and schedule initial appointments
  • Compile and verify referral information and communicate such information as needed.
  • Collaborate with agency staff to ensure that client needs are met in a timely fashion.
  • Manage the material assistance and outside referral list

Responsibilities of Case Manager-40%

  • Overall coordination of services to clients receiving multiple services, including but not limited to
  • Ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs.
  • Primary responsibility for developing, writing, implementing, and revising client service plans while receiving input from team members
  • Documentation of all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.

We offer an excellent benefit package including: 3 weeks paid vacation(adjusted upwards for longevity), 14 holidays, 10 sick days, 2 personal days -(prorated based on FTE) , single health insurance plan -(prorated based on FTE), fully paid pension (tax sheltered annuity), long-term disability, life insurance, business travel mileage reimbursement and job flexibility.

Salary range: $26,750 - $36,000 depending on education and experience prorated to FTE
